In order for us to issue a license for a California marriage certificate to you we will require:

a government-issued identification
a copy of any document related to the end your last marriage or domestic partnership agreement within the last 2 years. This includes:

                -a copy of your divorce decree
                -a copy of your annulment decree
                -a copy of the death certificate of your former spouse
                -a copy of your termination of domestic partnership agreement
